I hereby grant HemCon Medical Technologies, Inc. ("HemCon") the right to use my name and voice, together with my endorsement or testimonial (whether written or oral) in all media and in all types of advertising and promotion for HemCon's products and services.

I agree that HemCon owns any and all rights, including copyright and any other intellectual property rights, in and to all material taken and used by HemCon, its authorized representatives or contractors, which contain my voice, endorsement or testimonial. I agree that no advertisement or other material need to be submitted to me for any further approval. I hereby release HemCon and its authorized representatives and contractors from any liability arising out of any distortion or illusionary effect resulting from the publication of my voice, endorsement or testimonial.

I hereby certify that my endorsement or testimonial of the product or service is true and accurate and reflects my honest belief in the quality of the product or service that I am endorsing. I have no affiliation or connection with HemCon. My testimonial is given of my own free will and I have not been paid a monetary sum for doing so.

I hereby certify that I am over twenty-one years of age and competent to contract in my own name regarding the subject matter of this release. I have read this release before signing and I fully understand its contents.
